Consultations are individualized based on your level of health and if you have an existing healthcare directive. I am a Respecting Choices® certified facilitator at the First Steps®, Next Steps® and Advanced Steps® levels. I use their evidence-based approach to lead conversations for healthy individuals as well as those with chronic or active illness, including decision makers for loved ones with dementia.  

Advanced Care Planning Consultations include 

  • Education on choosing a healthcare agent

  • Exploratory conversations and informed treatment decisions

  • A written summary of conversations to use as a guide

  • Education on completion and distribution of the healthcare directive

  • Follow-up to ensure completion ( up to 6 months)  

Advanced Care Planning Revision 

For individuals or couples who need to revise an existing healthcare directive. Recommended if it has been over 10 years or if you have had a new diagnosis, a decline in health, a change in marital status, or death that has impacted you. Time and price will depend on multiple factors and will be determined during a phone consultation. ( minimum 1 hour for $100)  

Young Adult Advanced Care Planning Workshop  

One-time group meeting designed as an introduction to advanced care planning for young adults over 18. This is an interactive workshop to learn self-advocacy for healthcare, and participants will have an opportunity to complete a legal healthcare directive by the end of the workshop. Advanced Care Planning for healthcare directive
(individual or couple)  

For individuals or couples who have never had a healthcare directive or completed one without any conversations.

Two Meeting Structure

  • Initial consult ( 1.5 - 2 hours*) includes holistic conversations

  • Second consult ( 1- 1.5 hrs**) complete any decisions, review summary and healthcare directive, and next steps

      

The presence of the healthcare agent is required at one of the meetings. Price includes both consults, a written summary, follow-up, healthcare directive form, wallet card, and any decision-making materials.
($250 ind/$400 couple )

*: Approx time and will depend on the individual/couple and level of health. **: Time will depend on the stage of completion of the healthcare directive form and if the healthcare agent was present at the first appointment 

Advanced Care Planning Coaching

For family member(s) who want to learn their loved one’s goals and to feel confident in their ability to advocate for them. Rather than directly facilitating with your loved one, I coach you on how to have the conversations. (Loved one is not present.)

One meeting with one or more family members (1-1.5 hr)

Price includes consultation, healthcare directive forms, and decision-making materials ($150)

Senior Care/Living consultation will be added to the second advanced care planning meeting, or can be completed in a separate meeting.
($125 is added to the cost)

ACP Revision PLUS Senior Care/Living Consultation

Consults are completed either in one meeting or in separate meetings. Frequency, length, and cost will depend on the extent of the revision and will be determined in a phone consult.
(approx 2 hours, $225)

ACP Coaching PLUS Senior Care/Living Consultation

Consults are completed either in one meeting or in separate meetings if the loved one is present for the senior care/living consult. ($250)
