Senior Care/Living Consultation PLUS ACP for individual/couple
Senior Care/Living consultation will be added to the second advanced care planning meeting, or can be completed in a separate meeting.
($125 is added to the advanced care planning cost)
Senior Care/Living Consultation PLUS ACP Revision
Both consults are completed either in one meeting or in separate meetings. Frequency, length and cost will depend on the extent of the revision and will be determined in phone consult.
(approx 2 hours, $225)
Senior Care/Living Consultation PLUS ACP Coaching
Both consults are completed either in one meeting or in separate meetings if the loved one is present for the senior care/living consult.
